Once your first appointment is booked, you will receive an intake form to complete via an EMR system called Jane app. Please complete this form as soon as possible and it must be completed within 24 hours of your scheduled appointment.
A credit card is required to hold and confirm an appointment and the information is encrypted and stored securely. Please note that 48-business hours notice is required for any appointment changes to avoid a cancellation fee. Dr. Eberhard and other patients waiting for an appointment appreciate your cooperation with this policy.
Preferred methods of payment include direct billing, e-transfer, and cash but the credit on file may be used for payment if preferred.
If you would like to be added to Dr. Eberhard’s wait-list/cancellation list, please reach out and specify your preferred time of day for an appointment or add yourself to her wait-list on the booking page.
Things to have available during your first appointment, if possible, include:
A copy of any recent blood work or imaging reports
Any current medications or supplements
